• Vision exam for distance, peripheral vision, & color identification as related to traffic signs & signals
• Hearing exam using the “forced whisper” assessment
• Measurement of blood pressure, pulse, oxygen saturation level, height, & weight
• Urinalysis for Specific Gravity, Protein, Blood, & Glucose
• Physical examination, including the assessment of your:
  • General appearance
  • Eyes
  • Ears
  • Mouth & throat
  • Heart
  • Lungs & chest (breast examination not included)
  • Abdomen
  • Vascular function (circulation)
  • Genito-urinary (includes hernia check)
  • Extremities (includes assessment of flexibility, ease of movement, grip & muscle strength)
  • Spine (includes assessment of flexibility & ease of movement_

Based on the results of your examination, additional testing may be required for completion of the DOT FMCSA Medical Certification Examination.

• Arrive at least 15 minutes ahead of your scheduled appointment time so you can complete the required documentation for your examination.
• Wear comfortable clothing that allows for completion of the required examination assessment & associated activities.
• If you wear glasses or hearing aids, please wear them to your examination appointment
• Bring a list of ALL the medications & supplements you take, including any non-prescription or over-the-counter medications & supplements.  The list must include the following information:
  • The full name of the medication or supplement
  • The dose you take & how often you take the medication or supplement
  • The reason you take the medication or supplement
  • The name of the prescriber, including their phone number or office address
• If you have Diabetes you must bring a copy of your most recent Hemoglobin A1c test results & your recent blood sugar records (90 days)
• If you have Sleep Apnea you must bring a copy of your CPAP or BiPAP compliance report for the last 90 days, & the name of the provider following you for your sleep apnea, including the provider’s office address & phone number.
• If you have had any of the following Heart Conditions you must bring a current letter (1 year or less) from your cardiologist stating you are safe to drive a DOT or commercial vehicle.
  • Heart stent
  • Heart valve replacement
  • Pacemaker
  • Open heart surgery
  • Cardiac bypass surgery
  • Heart attack
