WCHC offers skilled nursing care for patients recovering from illness, surgery, or injury who no longer require an acute level of care in the hospital but aren’t ready to be discharged and on their own at home.
Our interdisciplinary team of hospitalists, physicians, nurses, and healthcare professionals work closely with each patient to ensure they reach their maximum potential for independent living. Our goal is to provide you with excellent care while working towards a smooth and safe transition to home.
For skilled care to initiate, you must have been hospitalized for three consecutive nights under an acute level of care. Reasons you may qualify for skilled care may include, but are not limited to:
- Physical therapy
- Occupational therapy
- Speech therapy
- IV antibiotics/infusions more than once a day
It’s important to note that Medicaid, Medicare, and private insurers each have their own guidelines regarding coverage of skilled nursing care.
In addition to 24-hour nursing care under the supervision of a registered nurse, our skilled nursing care unit provides a range of services as needed:
- Individualized assessments and care plans
- Administering medications and treatments as prescribed by your physician
- Rehabilitation services: physical, occupational, respiratory, speech, and pharmaceutical
- Chronic disease education
- Wound care
- Social services
- Spiritual services
- Specialized food and nutrition needs
We understand that the smallest details can make the biggest difference in your recovery. Our amenities include:
- Private rooms featuring a family area, day bed, and large windows with views of our beautiful landscaped exterior
- Large private bathroom
- Indoor and outdoor family gathering areas
- In-room cable television access
- Activities and recreation
- 3 daily meals with a bedtime snack
- Laundry (including personal items)
- On-call Chaplain
