Our interdisciplinary team of hospitalists, physicians, nurses and healthcare professionals work closely with each patient to ensure they reach their maximum potential for independent living. Qualifications

For skilled care to initiate, you must have been hospitalized for 3 nights under an acute level of care. Reasons you may qualify for skilled care may include, but are not limited to:

• Physical Therapy
  
• Occupational Therapy
  
• Speech Therapy
  
• IV Antibiotics/infusions more than once a day
  

WCHC Skilled Care also provides:

• 24 hour nursing care under the supervision of a registered nurse
  
• Medications and treatments administered as prescribed by your physician
  
• Individualized case management for all patients
  
• Rehabilitation services- physical, occupational, speech, respiratory, and pharmaceutical therapies
  
• Chronic Disease Education
  
• Social Services and spiritual services
