Clinical Pharmacy Services
Sarah FitzPatrick, PharmD, BCACP,
is WCHC’s Clinical Pharmacy Specialist, serving as a medication expert available for both patients and providers. Sarah is board-certified in ambulatory care pharmacy through the Board of Pharmacy Specialties. Ambulatory care pharmacists practice in outpatient care settings to provide education and collaborative management of chronic disease states together with patients and their providers.
Your provider may choose to consult the clinical pharmacy specialist to be a member of your specialized care team. Currently, Sarah offers a consult service for management of anticoagulant medications (aka “blood thinners”) like warfarin and Eliquis® and can even check your INR right in her office! She also has a general consult your provider can place to help with management of other chronic disease states or any other medication-related issue as needed.
Patients are also welcome to self-refer for clinical pharmacy services if they feel they need help with their medications! If you have questions about the medications you take, feel that you take too many medications, or need help managing a complex medication regimen, Sarah would be happy to meet with you to work through these concerns.
Our clinical pharmacy specialist is also responsible for meeting with all patients who will be having a joint replacement surgery at Washington County Hospital & Clinics before their surgery to give instructions on how to manage medications around the time of surgery.
